NHS GP Pathfinders (GPC): A new reproducible urban model for primary care at scale.
I spoke about our work at GP Pathfinder Clinics, a primary care organisation serving more than 110,000 patients in Brent. We discussed how we changed access and triage, tested the service with patients and kept reviewing it after launch.
