GP Pathfinder Clinics

Using data and coordinated recall in diabetes care

This was a multidisciplinary GP Pathfinder Clinics programme. My contribution as Clinical Lead GP and Clinical Safety Officer sat alongside the diabetes clinical lead and the wider team.

The work

The team used structured data, coordinated recall and targeted intervention to improve long-term-condition care.

Reported results

Over 12 months, the programme reported blood sugar target attainment rising from 65% to 75%, foot-check completion from 78% to 87%, and statin use in the reported high-risk group from 75% to 90%. These are team and organisational results.

Recognition

The multidisciplinary team at GP Pathfinder Clinics won the 2025 Quality in Care Diabetes Award for Improvements in Diabetes Care Using Data.
