Questionnaires · Low code
Ask about military service and follow up reliably
Use an Accurx questionnaire to ask about service history, then make sure someone reviews and acts on every reply.
What you’ll have at the end
A tested questionnaire and a clear process for reviewing replies.
How to begin
- Agree a narrow identification and contact purpose, rather than diagnosis. Check access to Accurx Desktop and Questionnaire Plus: custom questionnaires are a paid capability.
- Draft short questions about past service, whether contact is welcome and safe contact preferences. Explain who reads replies and include your practice’s approved urgent-contact instructions.
- In Accurx Desktop, open Manage Organisation → Questionnaires. Build the form, assign an owned response team or folder and confirm any SNOMED mapping with the clinical lead.
- Save for testing with an approved test patient. Check invitations, response options, routing, record saving and handover when staff are away.
- Pilot a small cohort, retain a telephone alternative and audit unresolved replies. Review clinical answers before coding or action. Increase volume only when the team can manage the workload.
What to measure
- Outcome: newly identified veterans receiving the agreed follow-up.
- Process: replies reviewed within the locally agreed interval.
- Balancing: unreviewed backlog, incorrect coding and digital/non-digital response differences.
Before using real data
Custom questionnaires have no branching, automated scoring or automatic reminders. Free-text responses cannot batch auto-save. This is a supported workflow, not a fully automated veteran review.
